Connecting families and individuals with the right services is becoming increasingly important as governments seek to tackle the many social determinants that impact health care outcomes and overall spending. The State of Idaho has employed an innovative approach to aggregating both government and community services through a single platform and is streamlining the process of matching citizens with the right support through its 'Live Better Idaho" program. We will share the origins of this initiative as well as the technical, business and political hurdles that were overcome in helping many Idahoans to "live better".
